New Requirements for Identification for your Clinic visit

New government regulations (Section 114 of the FACT Act) require that you present 3 separate forms of identification when receiving treatment at the clinic in order for us to file any insurance claims for your visit. We also have to have a copy of a Photo ID (ie. Driver's license) or a picture entered into your medical record. All the ID's must have the address we have on record for you. The 3 forms of identification include:

  1. Driver's License or other photo ID
  2. Insurance Card- needs to match Driver's License or Photo ID
  3. Other: Social Security Card, Military ID Card, Birth Certificate, Current Auto Insurance card or policy, Utility bill, Credit Card bill- any address on these must match the address we have on file and agree with the Photo ID.

Note that once you are an established patient, we only need to see your Photo ID/Driver's License and current medical insurance card every 6 months.

Also, if you arrive without all the required ID's, we will still treat you, but will not be able to file any insurance claims until you present the required 3 forms of ID. This will leave you responsible personally for the cost of treatment.
