Please be sure to
bring your insurance card, picture id, list of your medications
and pharmacy telephone number with you to your appointment.
Will you be a New Patient?
New patients
should arrive 30 minutes early to complete new patient information.
All new patients must have a physical exam to become an established patient, so you
will need to include this for your first visit.
IMPORTANT: New regulations require 3 forms of identification. For more

Medical History Form
and a
Patient-Information-Worksheet.
We highly recommend that you print
and fill these out ahead of time to bring with you at
the time of your appointment. It needs quite a bit of information that you
might not have readily at hand when you are already at the clinic. Please do not fax this to us.
