There are forms you can print and fill out prior to your visit, or submit them directly from this website(or the Practice Portal), saving time when you come in.
We take pride in our patient friendly primary care/internal medicine practice and hope your visits to us are as pleasant as we can make them. Treatment at The Light Clinic PC is provided by Primary Care Physicians as well as Family Nurse Practitioners working with them.
